Students who are participating in blended learning must submit a consent form for COVID testing.
The deadline to submit the consent form is Friday, January 15.
If we do not receive the signed consent form by 3 p.m. today, your child will automatically be switched to full remote learning and will not be able to attend classes in person when our school reopens.
The form is attached below. You can also complete the form through your NYCSA (My Student) account online (https://mystudent.nyc/).

Remember: Submit the form January 15 by 3 p.m. in order to have your child continue in blended learning.
