As you may have heard, the DOE has issued a policy that all students returning to schools to participate in blended instruction must submit a signed consent form for COVID-19 testing. Although our schools are not reopening on December 7, we are encouraging parents to sign and submit an updated form to make the transition smoother when we do.
Please note that testing will still be random but will now occur weekly, rather than monthly, as before. The consent form, a medical exemption form, and student with disabilities exemption form are attached below.
